In your February 2021 article “Drugs That Cause Magnesium Deficiency,” you included thiazide diuretics (such as chlorthalidone [available in generic only] and hydrochlorothiazide [MICROZIDE]) and loop diuretics (such as furosemide [LASIX]) on the list of drugs that can cause magnesium deficiency. Does the potassium-sparing diuretic spironolactone (ALDACTONE, CAROSPIR) also cause this problem?

Unlike the diuretics mentioned in our February article, spironolactone does not cause magnesium deficiency and low blood magnesium levels. In fact, results of one randomized clinical trial found that spironolactone treatment helped the body maintain normal magnesium balance in patients with heart failure.[1] Therefore, patients taking spironolactone do not need to be concerned about this drug causing low blood magnesium levels.
